Lady Macbeth dies offstage. Her death is reported by Macbeth's servant Seyton with the simple words, "The Queen, my lord, is dead." In the very last scene, Malcolm offers his own view of the event, saying:

...this dead butcher and his fiend-like Queen,

Who, as 'tis thought, by self and violent hands

Took off her life...

Meaning that Malcolm believes Lady Macbeth commited suicide.
